Membership terms, in plain English

These are the terms of the Proceed Dental Savings Plan offered by the dental offices of Dr. Keerthi Senthil, DDS MS (Implants Guru, Rancho Mirage and Smile Guru, Cathedral City). By enrolling, you agree to them.

What Proceed is — and isn't

Proceed is a discount membership program between you and our dental offices. It is not dental insurance, and if you carry dental insurance, your insurance applies first — the membership is secondary to it.
Member savings apply only to treatment performed at our two offices. They do not apply to outside specialists we may refer you to, hospital care, IV sedation or general anesthesia, or to treatment covered by workers' compensation, disability coverage, or a legal claim.

Your membership year

A membership lasts 12 months and is paid in full when you enroll. It becomes effective the day full payment is received.
Renewal is due at the end of your 12-month term. If your membership lapses and you want to rejoin, a $50 reinstatement fee applies.
If you enroll with auto-renewal, the card on file is charged on your renewal date. You can cancel auto-renewal any time before that date by calling the office.
Membership fees may be reviewed and adjusted from year to year; you'll always see the current fee before you renew.

Using your benefits

Included services (exams, cleanings, X-rays, fluoride) are yours to schedule; unused visits don't roll over and aren't refunded.
Member savings are taken off our standard fees at the time of treatment. Payment is due when services are rendered — if an account isn't kept current, member pricing doesn't apply.
If you pay through third-party financing, a 10% administrative charge is added to the financed amount.
Memberships are personal: they're non-transferable and family members can't be substituted for one another. Savings can't be combined with other memberships, discount plans, or office promotions.

Changing your mind

If you cancel within 30 days of enrolling and haven't used any services, you receive a full refund.
If you've already used services, your refund is reduced by the regular (non-member) value of the services you received.
